Poor R wave progression (PRWP) is often a sign of prior MI, though there are numerous possible etiologies. Normally, the R wave should progressively become more positive/taller throughout the chest leads from V1-V6. In anterior MI, this poor progression is similar to Q wave formation. The actual definition is by R wave being 3mm or less in V3. It is important to ensure that the leads were placed appropriately on the patient, however.
