DeWinter's T waves are a variant of hyperacute T waves (occasionally called a STEMI equivalent) due to occlusion of the LAD and, therefore, are found only in the anterolateral leads. It is a very rare finding in anterior OMI's, but is still under-recognized. This pattern may or may not evolve into a typical OMI on EKG.
The anterior leads show upsloping ST depressions (>1mm) into a hyperacute T wave without ST elevations. Patients also have subtle ST elevations in aVR.
